OAWA Ontario Winter Games Travel Request form

Please download and fill out this form and return it per the instructions on the form to request travel for the Winter Games.  DO NOT SEND THIS FORM TO THE GAMES.  Please return it to the OAWA as instructed on the form.  Only those who qualify will receive travel funding.  Funding will likely not arrive until April.
